Adult Mental Health Targeted Case Management
We provide Adult Mental Health Targeted Case Management (AMH-TCM) services to help adults with serious and persistent mental illness (SPMI) access the medical, social, educational, and vocational services they need. These services include functional assessments, individualized support planning, referrals, coordination of care, and ongoing monitoring to ensure continuity and effectiveness.
